How to use create a survey with the Spreadsheets:

  1. Go to docs.google.com
  2. Register and/or login to Google.
  3. Click Upload. upload1
  4. Download the template and Browse to the download location OR enter the URL for the template.
  5. Type in a name for the Survey (i.e. NETS Self-Assement).
  6. Click create.
  7. Once uploaded click on the Form menu.
  8. Select Create a form.upload2
  9. Copy and paste the directions for the template in the direction box of the form if needed.
  10. Change the question types to fit your needs (default question types are all text).
