These Microsoft Excel files below were created to provide you quick access to surveys you can administer in your school or district using Google Forms (a subset of Google Docs). For best results, log into Google Docs prior to accessing the templates.

Classroom Referral, Dean's Office, and Behavior Intervention Form- By Youssef Elias

Use this form from any computer or PDA with Internet connectivity by sharing the URL with your teachers. No more paper referrals for students to take with them to the office. Once a teacher completes the form online, office staff will have access to the behavior. Or, clerical staff can enter data from the paper onto the digital form. This form provides an easy way to search for students, teachers who submitted referrals, and quick access to common misbehaviors.

School Climate Survey -- Analyzing Teaching and Learning, Safety, Relationships, and Institutional Environments- By Youssef Elias and Jason Borgen
Survey three different stakeholder groups, students, teachers, and parents, to access data relating to school climate. You will see where perceptions lie within each stakeholder group and make a plan to improve school climate. The data dissagregates into four areas relating to school climate: teaching and learning, safety, relationships, and institutional environments. These areas correlate to research published by the National School Climate Center.
